15 minute consultation to discuss your health goals.
This consultation is an opportunity for you to tell me about your symptoms and health goals and ask any questions you may have about nutritional therapy. I will tell you about my philosophy and ask you some questions about your expectations of the process, and your needs in a client-practitioner relationship.

Together we will decide if you are a good candidate for the nutritional therapy process, and we will move on to the intake scheduling process.
​
In some cases, I may not feel that I am the best practitioner for you, based either on the type of care you require or your readiness to participate in a functional nutrition process. In this case I will make a referral or provide you with resources that best suit your needs.
Comprehensive

Once we have decided to work together, this is the first step for all new clients. Prior to booking your appointment I will ask you to complete the following paperwork:

After completing and submitting all of your new client paperwork, we will schedule a time for your first appointment. Local clients are encouraged to meet in person. For long-distance clients, the appointment will be conducted through Zoom, which I will send a secure code at the time of the appointment.
In this appointment, I will:

Review and analyze your intake paperwork

Conduct a 90-minute interview with you reviewing all relevant symptoms, health history, and lifestyle factors

Generate a comprehensive written protocol for you, including:

Customized nutrition, lifestyle, and supplement recommendations

Supplementary resources such as meal templates, shopping lists, books, YouTube videos, worksheets, etc.

Motivation and behavioral strategies for accountability and stress management
-
Recommendations for functional lab work
-
Referrals when appropriate

All appointments include a 10 minute follow-up phone call, ideally scheduled halfway between your last appointment and your next. I highly encourage all clients to take advantage of this included service to seek clarification, hold yourself accountable, and increase your likelihood of success with your current protocol. If a six- or twelve-week plan is chosen 60 days after initial intake questionnaire was complete, a new questionnaire will need to be completed with any additional charges to apply.

After the comprehensive appointment is complete, we dive deeper into a twelve-week customized plan for you.
This plan includes:

Customized meal plans are designed each week specifically tailored with you in mind.

Scheduled Zoom call for approx. 10-20 minutes each week to ensure you are staying on track, answer all questions you have for week and discuss anything pertinent to you and your journey.

You will receive weekly motivation, tips and tricks, recipes, and other fun resources to help you stay focused.

Discounts on medical-grade supplements (up to 20% on first-purchased recommendations)

PAYMENTS
I manage my weekly caseload to ensure you receive the highest quality of care. For this reason, you will be asked to reserve your package at time with a credit card number. If you choose the 12-week plan, we can break this up into three easy payments charged to the credit card on file in three installments.

Credit/Debit or Venmo is our preferred method of payment for first payment and payment thereafter.
​
Itemized invoices will be provided for all appointments. I do not participate with insurance plans and cannot provide insurance billing codes for reimbursement.
​
Late cancellations, (less than 48 hours in advance of the appointment) will be automatically charged a $50 late fee. Missed follow up phone calls will not be charged but will not be rescheduled. In the case of a no-show or late cancellation your card will be automatically charged the late cancellation fee.
